Casual: for the story

Most enemies are weaker and hit softer, and ammo is easier to find. Aim assist is on by default for both Grace and Leon. Puzzles and item spots match the Standard modes, so you are not missing content.

Standard: Modern or Classic

Both versions give the same enemy damage and health. The difference is saving. Modern autosaves often, so you rarely lose progress. Classic brings back the old rule: Grace needs an Ink Ribbon to use a typewriter. You can craft ribbons from tins using infected blood, and your small inventory becomes a real puzzle.

Insanity: the post-game test

Insanity is described as the toughest option, with scarce resources and heavy damage. Many enemies can take more than half of Leon's health in one hit. It unlocks after you beat the game, and one guide says you must defeat every boss. Some items sit in new places, so memory of the first run only helps a little. It also earns more Challenge Points for unlocks.
